To submit a new order, will search for a specific Court, Judge or Location via the Search tool.   When the correct search result is displayed, click on the text to open the correct order form.  In the example below, I am searching for "indiana", and selected "Indiana Tax Court":

If there are no search results, not to worry, you can still submit an order and we'll sort out the Jurisdiction details later.  In the example below, I am searching for "Demo Indiana" which is not a real jurisdiction, so when I click "Request Now", I am taken to an order form for the new "Demo Jurisdiction" (see video below):
